Prior to your appointment
Please complete your registration and health history forms from home prior to your first appointment, or arrive at least 15 minutes early to complete them in office.
At your first appointment
Your first appointment will be a consultation, where we will review your dental and medical history, your treatment and sedation options, and address any questions or concerns you may have. If we need additional x-rays, we will take them at this time. We will then schedule the appointment for your oral surgery procedure.
- All patients under the age of 18 will need to be accompanied by a parent or legal guardian.
- A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink eight hours prior to your surgery. You will also need an adult to drive you home.
- Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
Please bring with you
- Your surgical referral slip and x-rays, if available
- A list of medications you're currently taking
- Insurance cards

To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. They are well informed and up-to-date. Please bring your insurance information with you to the consultation so that we can expedite reimbursement.

Payment Options
We accept Visa, Mastercard, and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Cancellations
We require at least 24 hours notice to avoid cancellation fees.
